Please complete this form as soon as you are able after your wedding. Once submitted, please proceed to the additional tasks below and complete all applicable updates for your employment, benefits, and donors.

Some of the tasks in this list may take a while to complete. Please continue to proceed through the checklist while you are working on the various tasks listed to ensure the full process is completed promptly. 

Details: Completing a Release of Personal Information grants Reliant permission to release certain information to your spouse if you wish. You can also choose to designate your spouse as your emergency contact.

Medical

Details: If you are covered by Reliant-offered medical insurance, you must fill out a medical insurance form if your spouse will or will not be added to your plan. You may also want to look at your HSA Contribution amount. You can update this amount by completing a new Cafeteria Form.

Timeline: Changes can be made up to 60 days from the date of your marriage for domestic medical insurance with GuideStone or 30 days for the International medical plan with Aetna to make any changes to your medical insurance. Coverage starts retroactively to the date of your marriage. Changes must be submitted to Reliant 3 business days before those deadlines to allow time for Reliant to process your changes.

Dental

Details: You can choose to add your spouse to your dental coverage.

Timeline: You have 30 days from the date of your marriage for domestic Dental insurance with RSLI or for the International medical plan with Aetna to add your spouse. Dental insurance for internationals is bundled along with their medical coverages listed above. Changes must be submitted to Reliant 3 business days before those deadlines to allow time for Reliant to process your changes.

Vision

Details: You can choose to add your spouse to your vision coverage. To elect this coverage, please submit an updated Cafeteria Form.

Timeline: You have 31 days from the date of your marriage to add your spouse for domestic vision insurance. Vision insurance for internationals is bundled along with their medical coverages listed above. Changes must be submitted to Reliant 3 business days before those deadlines to allow time for Reliant to process your changes.

Life Insurance

Details:  You can change your voluntary life insurance coverage for yourself or your new spouse. You can also update the beneficiary for both your all Life insurance coverage. Changes must be made within 30 days of the marriage or during Open Enrollment. Changes must be submitted to Reliant 3 business days before those deadlines to allow time for Reliant to process your changes.

Details: You'll need to change your support goal to correctly raise toward the increased coverage level.

To change your legal name on file with Reliant for Payroll and Tax purposes, please submit the New Social Security Card Form linked here with a picture of your new Social Security Card that shows your new last name. This form utilizes DocuSign which is a secured platform. Once you submit your new Social Security Card, Reliant can update your name in its systems. For questions, please email payroll@reliant.org

Please note that Pay Stubs, W2s, and income verification requests will be issued using the maiden name on file until Reliant has received a new social security card.

Details: If you are combining to your spouse's bank account or starting a new one, please let us know where we should deposit your paycheck.

Details: Some benefits can be paid out to a beneficiary. You can update that beneficiary at any point.

• Notify your donors of your marriage in a prayer letter
• Reach out to check donors personally, since they manually add their name to checks.
• If your spouse is a donor to the account you are responsible for, email donor.services@reliant.org to cancel that recurring gift.

If you are merging funds, also complete the steps below.

• If a donor is giving to both accounts you are individually responsible for, ask the donor how they would like to update their giving and notify donor.services@reliant.org of the change.
• Notify your donors that you will be sharing a Reliant account, and gifts will be automatically transferred to the new account number.
