Reliant does not release personal information about its employees to third parties unless there is consent from the employee. Third-Party Designees may include parents, spouses, children, and others.
You may grant Reliant permission to release certain information to a third party by submitting this form for each party authorized to receive or access your personal information.
Further, Reliant must have contact information on file for someone to call in case of an emergency. Please use this form to update your emergency contact and indicate what information we can discuss with that individual.

Can we add a note to this page or something on the form, that helps missionaries know which program to choose from. Because I have gotten quite a few missionaries, that are under Fixed-Term putting US Churches, instead of Fixed-Term, and even a few Collegiate missionaries have trickled in. It could be they are not reading the form carefully. But I was wondering if there is something we could add, so that way it helps the liaisons save time, by not having to go into CRM and double-checking in the attributes section, or someone gets lost in the shuffle. Appreciate the feedback!

Mike Swann - true, or could we maybe do Collegiate - Collegiate Network and GCM and then US is - non-Collegiate, non GCM? Would that help, because I think people are still used to working with KZ vs. me.

The option would be Collegiate Church Network and GCC (not GCM), but unfortunately there really isn't a GCC anymore. It was a network but is not now, which is why you hear me say "historcally GCC". I think in my world I just need to do a better job of letting people know to click Collegiate. I'll be sending out info to these churches soon to reiterate that.

I've also been thinking about a linked Solomon page that says (Which Program Team am I in?) that we could link places to help people remind themselves. Could be a project we could work on.
