⚠️ Open Enrollment: October 21 – November 4, 2026
Changes made during Open Enrollment will take effect January 1, 2027.
If you make benefit contribution or deduction changes on or after October 21, 2026, you must complete two separate forms:
- One form for 2026 changes; see below
- One form for 2027 elections 2027 Cafeteria Form – Fill out form
Important: 2026 contribution elections do not carry over to 2027. New elections must be submitted for the 2027 plan year.
