Due to some significant technical difficulties, 2 companies were used for filing the 2022 Employees who were benefits-eligible for a month or longer in 2022 are required to receive a 1095-C forms form for Reliant. We apologize for the inconvenience and confusion this may cause. We are working with our long-term tax form partner, Greenshades, to enhance their filing services but for 2022 they were only able to produce online access to the 1095-C forms. The majority of benefits-eligible employees from 2022 will receive an email notification with online access to download a PDF of their 1095-C Form from Greenshades. A minority of benefits-eligible employees from 2022 had forms that were unable to be submitted using Greenshades. Those forms were processed with our previous tax partner, Tax Bandits. For these employees, they will receive both an email notification with online access and a printed form in the mail.filing their taxes. This form is entitled, “Employer-Provided Health Insurance Offer and Coverage.” In compliance with IRS requirements and deadlines, Reliant has filed these forms with the IRS today, and recipients should have received an email with instructions for electronic access to their forms. This email will come from one of two sources. Some employees will have received an email from barb.seckler@reliant.org with access to their form via the Greenshades website. Other employees’ forms will be distributed from another company, Tax Bandits. (We used two separate companies this year due to our transition between systems.) Please see the instructions below for accessing your 1095-C form online, based on the email you received from one of these 2 providers. These instructions will be updated with more specific instructions once the forms are available online. Currently, these forms are still processing status with the IRS. Tax bandits will send these 1095-C recipients an email. Within the email, they will be given a link to click, and then they'll be asked to enter their registered email address and the last 4 digits of their Social Security Number (SSN). An OTP is then sent to the individual. Upon entering the OTP, the forms can be accessed. |

When you login to Greenshades Online, you will be asked to consent to receive an electronic 1095-C in place of a paper 1095-C, and a verification code will be generated that you will need to type in.
