Eligibilty 

Reliant offers optional vision insurance through EyeMed to all U.S. field staff, regardless of the number of hours worked. Both full-time and part-time employees are eligible to enroll.

Coverage is effective on the employee's date of hire, provided the employee enrolls within the applicable enrollment period. Eligible employees may also enroll eligible dependents. For more information on adding a dependent to coverage, see Adding a Dependent to Insurance.

Enrollment

Enrollment in EyeMed Vision Insurance is completed annually through the Reliant Cafeteria Plan.

Employees generally have 30 days following their date of hire or a qualifying life event to enroll in or make changes to vision coverage. Otherwise, enrollment changes may be made during the annual Open Enrollment period.

Premiums

Vision insurance premiums are paid by the employee on a pre-tax basis through the Reliant Cafeteria Plan.

Monthly premiums are deducted from the employee's paycheck.

Enrollment in the Vision plan requires submission of a Cafeteria Form. This form must be submitted each year during Open Enrollment. If the form is not submitted, Vision coverage will end on December 31, 2025.
The Cafeteria Form link will become active at the start of Open Enrollment. 

Benefits

  • Annual lens and bi-annual frames for glasses after a $25 copay.
  • Contact lens coverage up to $120/year, $0 copay.

  • Field staff can also choose to process an annual eye exam separately through GuideStone. 
  • A tax-free benefit means that you pay no Federal income taxes or FICA taxes on the premiums paid through the Reliant Cafeteria Plan.
