Please do not send your requests to member services or directly to Aetna. All requests should come through employment services.
Please allow up to 2 weeks for these requests to be processed.
At times you may need to request a Verification of Coverage or Foreign Insurance Certificate for travel to foreign countries or to provide to embassies upon entry.
A Verification of Coverage can provide proof of coverage for the following:
- Insurance with your effective date
- Dependent coverage with the effective date
- Inpatient hospitalization
- Outpatient care
- Maternity care
- Prescription drug
- Medical evacuation
- Repatriation of remains
Verification of Coverage or Verification of Benefits
When you need to make a request for a Verification of Coverage/Benefits, please follow the instructions below:
Determine what kind of verification letter you are wanting to request.
Email your request to benefits@reliant.org.
You can send one request for your whole team, but make sure to list out each worker's name within the request.
Also, make sure to include the country where you are going.
- Name - Country
The benefits team will then process your request and send it to our representative at Aetna.
Once the benefits team receives the certificate from Aetna, they will send it to your Reliant e-mail address.
Foreign Insurance Certificate Request
When you need to make a request for a Foreign Insurance Certificate Request, please follow the instructions below:
Email your request to benefits@reliant.org for a Foreign Insurance Certificate. Please attach the certificate provided by the embassy and the corresponding passport number for each individual.
The benefits team will process your request and send it to our representative at Aetna
Once the benefits team receives the certificate from Aetna, they will send it to your Reliant e-mail address.
Below is an example of a Verification of Coverage Certificate
Please note: A request can be made to Aetna for the Customer Name not to say "Reliant Mission" if needed. Feel free to include that in your email request for the Verification of Coverage.
