Dependents of Reliant employees covered on Reliant's Domestic vision insurance plan (through LincolnEyeMed) are eligible for continuance coverage on Reliant's vision plans under the following circumstances:

The only exceptions that allow a change in the vision coverage are during Reliant's annual Open Enrollment period  (Mid October-Mid-November for coverage effective January 1st) or in the event of the death or divorce from the Reliant employee or if the Reliant employee became eligible for Medicare, in which case a change to the covered persons on the policy would be appropriate.
