Reliant employees covered on Reliant's domestic vision insurance plan (through EyeMed) are eligible for continuance coverage under the following circumstance:
- The benefits eligible (30 hours or more) Reliant employee exited employment with Reliant.
Dependents of Reliant employees covered on Reliant's Domestic vision insurance plan (through EyeMed) are eligible for continuance coverage on Reliant's vision plans under the following circumstances:
- The benefits eligible (30 hours or more) Reliant employee exited employment with Reliant
- The Reliant employee becomes eligible for Medicare coverage (due to turning age 65 or another qualifying reason)
- In the event of the death of a Reliant employee
- In the event of divorce or legal separation from the Reliant covered employee
No, when switching to continuance coverage, there is NOT an option to change your selected vision plan (adding and dropping dependents etc). The coverage offered is the same coverage that the employee had prior to the loss in benefits eligibility.
The only exceptions that allow a change in the vision coverage are during Reliant's annual Open Enrollment period (Mid October-Mid-November for coverage effective January 1st) or in the event of the death or divorce from the Reliant employee or if the Reliant employee became eligible for Medicare, in which case a change to the covered persons on the policy would be appropriate.
