  • How to Enroll and Open a Health Saver Plan

    • Select a Health Saver Plan 
    • Select a rate for your Employee Contribution Rate to your HSA
      • Starting in 2017, missionaries use one form to choose the Health Saver Plan and to select the monthly contribution amounts for their Health Saver Accounts.  (If you are a new missionary, fill out the cafeteria plan enrollment form

        • Each year you must renew your request for monthly contributions to your HSA.  
        • Limits for Health Saver Accounts are issued by the IRS each year.  Be sure your combined Employer + Employee contributions are within the annual limit. 
    You will receive a debit card to use for your HSA.
    • Open your personal HSA Debit account 
    • Send Confirmation of your HSA account to HR@reliant.org 
      • Once you have requested for the HSA to be activated, send hr@reliant.org a screenshot of your request for HSA activation. Until Reliant receives notice of your HSA
    activation.  You have until the payroll deadline (20th of the month) to send in proof of the activation of your account in order to receive employer contributions. If you miss this deadline, you will not be able to retroactively receive the Reliant employer contribution but will be able to receive the contribution going forward. 
      • Debit Account activation, you are not able to receive any of your HSA contributions, neither the Employer portion, or the Employee portion. In order to guarantee that your first month's contribution is not lost, please send the activation by the 20th of the month.  If you are delayed, Reliant will help according to our capacity, but you may risk losing contributions to your HSA debit account. 

    • HSA Debit Cards will be issued by Bank of America
      • HSAs may have up to 3 Debit Cards for dependents in addition to the policyholder's debit card.
        • Dependents must be at least 16 years’ old.
        • Requests may be made by phone or online.
          • Highmark website (login as policyholder) > Spending > Debit Card > Dependents & Beneficiaries > GO>Debit Card > Order Debit Card.
      • Keep Records of purchases made with your HSA debit card, along with Doctor's notes for tax purposes.
