Open Enrollment Instructions:
If you want to enroll, change, or terminate your coverage for 2026, you can complete the 2026 Voluntary Life Open Enrollment Form.
Enrolling in new coverage
Good news! If you are a brand-new Voluntary Life Insurance enrollee, during 2026 Open Enrollment, you may purchase up to $50,000 in RSLI life insurance for yourself (and up to $10,000 for your spouse) without needing any medical exam. What a great time to consider ensuring you have adequate life insurance for yourself and your family. This is a very generous enrollment offer from RSLI since a medical exam is often required.
- If you or your spouse is being enrolled in coverage over the Guaranteed Issue Amount, the Benefits Team will contact you with further instructions, which may include completing a DocuSign power form from Reliance Matrix, our life insurance carrier.

- Age Restrictions May Apply: the above-listed increases are allowed for employees under the age of 70 and spouses under the age of 60. If you or your spouse are over these ages, these coverage amounts may be available, but additional forms and medical exams may be required.
- You may also elect coverage for children up to a maximum amount of coverage of $10,000. The coverage applies to children aged six months to 26 years.
Changing existing coverage
If you are selecting an increase to your existing Voluntary Life coverage, as long as you do not exceed a change or addition of $50,000 in coverage for yourself (employee) or $10,000 for your spouse, then no additional forms are required. You will still be limited to the annual maximum coverage of $100,000 for employee coverage and $30,000 for a spouse.
A special voluntary life application with medical questions and a possible medical exam is required if:
- You elect to add or increase coverage by more than $50,000 for an employee and more than $10,000 for the spouse.
- Your new total requested amount exceeds $100,000 for the employee and $30,000 for the spouse
