In the case of the covered Reliant employee's death, RSLI will request a copy of the Beneficiary form on file with Reliant as well as a *certified copy of the death certificate. Reliant will reach out to the employee's family/beneficiary to obtain the death certificate and will provide the beneficiary form on file to RSLI. Reliant will work with RSLI to process the claim and will correspond with the employee's family/beneficiary regarding the timing of the claim or any additional documentation that is needed for the insurance proceeds to be paid out. See Group Life and Voluntary Life Insurance for additional details of the Group Life coverage provided by Reliant. *If the death certificate is in a language other than Englsih, RSLI will require a translated certified copy of the death certificate. |
