The information provided through the Personal Ministry Report allows the MTD coach to receive a better idea of how and what a staff member is doing.

Your coach will likely send you a Google Form version of the Personal Ministry Report for you to submit weekly. If the online version is not available, you can download the Word document below.

    We already have a Google form for this that we use when we teach Coaches.
