UHA 600 Plan

The UHA 600 Plan has the following features:

  • No annual deductible
  • UHA pays 90% of physician services
  • UHA pays 90% of hospital and emergency room charges
  • UHA pays for 90% of maternity charges

UHA 3000 Plan

The UHA 3000 Plan has the following features:

  • Annual deductible $200 per person, $600 per family
  • Out-of-Pocket Maximum $2,200 per person, $6,600 per family
  • The participant pays a $12 co-pay for physician services
  • UHA pays 80% of hospital and emergency room charges
  • UHA pays for 100% of maternity charges

Eligibility

Employees are eligible for coverage (based on Hawaii's ACA guidelines) after they have worked 20 hours or more per week for 4 weeks.  This is DIFFERENT than Reliant's normal eligibility of employees working 30 hours or more having coverage offered the day of their hire.  

Enrollment

Employees must be enrolled within 31 days of hire.  Coverage always begins on the 1st of a month and always terminates on the last day of a month. If an employee has not reached their 4 weeks (of working 20 hours per week) by the 1st of the month after their hire, they will be enrolled for coverage the second month following their hire (starting on the 1st of that month), and that enrollment must occur within 31 days of their hire.  

Example #1 - someone hired August 1st - would be eligible for coverage  Sept 1st, and their enrollment must happen by August 31st

Example #2 - someone hired August 15th would be eligible for coverage October 1st, and their enrollment must be completed by September 15th

Qualifying Events:

  • Initial hire working 20 or more hours per week
  • Loss of other medical coverage due to turning age 26 or spouse/parents loss of employer coverage
  • Qualified Medical Child Support Order (authorizing the addition of a child to the plan)
  • Annual Open enrollment
