Dear Member,

Your IRS 5498-SA Tax Form has been generated for your HSA and will be available within 48 hours for you to view, download and print. If you are not opted in to electronic delivery*, your tax form will also be mailed to you.

You may access your IRS 5498-SA Tax Form by logging in to your account. Visit the website on the back of your Member ID card. Click Benefits and your Spending Account under Coverage, then click View my account details. Once on your personal dashboard click on your HSA balance in the My Accounts section, then Tax Forms under account Resources. Your tax statements are archived so you may go back and review and/or print if needed.

*To opt-in to future electronic delivery of your tax forms, log in to the website on the back of your Member ID card and click Benefits and your Spending Account under Coverage, then click View my account details.  Instructions to go green and opt in to electronic delivery will be located in your Personal Dashboard under Document Delivery Preferences. Click Tax Forms to select Electronic Only.
