2022 ACA Forms Information

Employees who were benefits-eligible for a month or longer in 2022 are required to receive a 1095-C form for filing their taxes.  This form is entitled, “Employer-Provided Health Insurance Offer and Coverage.” In compliance with IRS requirements and deadlines, Reliant has filed these forms with the IRS today, and recipients should have received an email with instructions for electronic access to their forms. This email will come from one of two sources. Some employees will have received an email from barb.seckler@reliant.org with access to their form via the Greenshades website. Other employees’ forms will be distributed from another company, Tax Bandits. (We used two separate companies this year due to our transition between systems.)

Please see the instructions below for accessing your 1095-C form online, based on the email you received from one of these 2 providers. If you did not receive an email or if you need further assistance accessing your 1095-C form, please reach out to us at payroll@reliant.org or contact your Liaison.  


Greenshades Online Access

Employees should have received the following email from barb.seckler@reliant.org on Thursday morning 3/2/22 (please check your spam folder if you do not see it). The email was titled: Reliant Mission, Inc. Employees - View Your 1095-C Online


Dear Reliant Mission, Inc. Employee,

This year our 1095-C forms will be available online. You can view your 1095-C at http://reliantm.greenemployee.com

Please click the link and enter the required information to log in and view your form. (You may need to create an account first if you did not already create a login when accessing your 2022 W2.)  The login and password are different than your other Reliant logins and are unique to Greenshades Online. 

When you login to Greenshades Online, you will be asked to consent to receive an electronic 1095-C in place of a paper 1095-C, and a verification code will be generated that you will need to type in. 

If you need assistance accessing this form, please reach out to your liaison or email us at payroll@reliant.org. Upon request, we can email you a PDF of this form or mail a printed copy of this form. Due to technical printing complications associated with this form, we had to send email access only for this form, rather than the mailed copy.  We apologize for the inconvenience.  


Tax Bandits Online Access

These instructions will be updated with more specific instructions once the forms are available online. Currently, these forms are still processing status with the IRS.

Tax bandits will send these 1095-C recipients an email. Within the email, they will be given a link to click, and then they'll be asked to enter their registered email address and the last 4 digits of their Social Security Number (SSN). An OTP is then sent to the individual. Upon entering the OTP, the forms can be accessed.
