The purpose of this page is to offer guidance as you prepare for your exit from a fixed-term program and Reliant employment. We encourage you to read through it and work through the checklist below.
At Reliant, we feel privileged to serve you and the many other workers God has planted around the world. We are grateful for the opportunity to have come alongside you on this journey. We will miss you, we will always be here for you, and we are excited to see where God takes you next.

What to expect when exiting:
Checklist
Please complete your Exit Survey.
We want to hear from you on your Reliant employment experience and how we can best serve future employees.
Check that we have updated contact information for you and inform us of any changes.
- You can find your address on file by looking at a recent paystub.
- Your Reliant.org account email address will revert to the personal email used in your application.
The contact information left with us upon your exit is what Reliant will use to communicate important year end tax statements.
If you need to update either of these items, please fill out the Contact Info Change Form.
*This form is only accessible when logged into your Reliant Google account. Reliant accounts and email addresses are deleted two months post-exit.
Please update all donors, both one-time and regular/monthly. Thank them for their generosity, and let them know you are exiting the program and employment with Reliant. Inform them that their giving will be stopped automatically and anything else you might like to share.
Prepare for insurance changes, if enrolled in a Reliant medical plan.
If you currently have medical insurance through Reliant, please complete this quick form to inform us about your decision concerning continuance coverage.Your Reliant health insurance coverage ends on the date of your exit unless otherwise requested through the above form.
Take accountability for any retirement investments.
If you've been contributing to your Reliant retirement fund, from your paycheck or through the employer contribution, login to your retirement account to become aware of the funds you are now responsible for. Kudos for planning for the future!
- Balances greater than $1000 will remain with GuideStone for you to keep there, rollover to another provider, or withdraw as you'd like.
- Balances of less than $1000 will be transferred to an IRA fund with Inspira. You'll hear from Inspira on options from there.

Tonya Bartels Kaitlin Hughes I would like to add to the "What to expect when exiting" section something about when their insurance coverage ends. Could you reply with the general rules of thumb for those who exit on any given day of the month?

Beka Page Kaitlin Hughes . Sorry, I am behind on this. I don't know if we want to add anything about GuideStone not pro-rating and how the charges work for a mid month exit. Also, there isn't anything mentioned about dental or vision, not sure if we want to include that information as well?

Coverage (for medical, dental, and vision insurance) ends on your exit date, meaning you have until your exit date to use your insurance for medical needs.

Beka Page continuance applies to medical, dental and vision (they can have continuance for any insurance they are enrolled in). Like medical, dental & vision insurance ends on the day of their exit (ie. 7/31/2025 would be the last date they can use their coverage). It isn't uncommon for fixed-term employees to waive medical, but be enrolled in dental and/or vision so they could elect continuance for dental and/or vision if that was applicable to their situation.

Caleb Hayworth Mini project if you could tackle this within the next few weeks. Will showed me an old FXTM Exit page (see attachment) that he's going to delete. I'd like to compare the old page (the attached PDF) and this current/active page. Are there things on the old page that aren't on this one? If so, do we want them here? If you could do that initial scan for discrepancies, and then either send them over or schedule a call and we can quickly work through what needs to be added to this page if anything.

Hey Beka Page I did a pretty thorough look at the old page and most of the content lives on this current page outside of the "This exit process is for Fixed Term residents and interns only" box and the first step "email your liaison" - the only other item was step 2 on the old page says to notify donors within 30 days of intended exit and this current page doesnt say a timeframe.
