If you have moved to a new state (and in some cases to a new city), you may need to fill out new tax forms. Please respond quickly to emails from Employment Services if they seek it seeks a new state (and potentially local) tax withholding form(s) if you are residing in a state/locality that requires a specific withholding form. Employees living in certain cities may be liable for additional taxes or forms to be filed directly by the employee employees with their local tax agencies.
