- Submit the Personal Information Change Notification Form to hr@reliant.org
- Update your 401K Plan address by logging into your account at principal.com
- If you are changing your financial institution you will need to send Reliant a new Direct Deposit Agreement. Completed forms must be received by Reliant by the 20th of a given month in order for the change to impact the next month's payroll and reimbursements.
- If you have moved to a new state, you will need to ask Reliant to send you a new state tax withholding form , if you are required by your new state to complete one. Reliant has responsibility for the employer-related federal and state filings , but not for any employee's federal or state tax forms or filings.
