- Submit the Personal Information Change Notification Form to hr@reliant.org
- Update your 401K Plan address by logging into your account at at principal.com
- If you are changing your financial institution you will need to send Reliant a new Direct Deposit Agreement. Completed forms must be received by Reliant by the 20th of a given month in order for the change to impact the next month's payroll and reimbursements.
- If you have moved to a new state you will need to ask Reliant to send you a new state tax withholding form, if you are required by your new state to complete one. Reliant has responsibility for the employer related federal and state filings, but not for any employee's federal or state tax forms or filings.
