Where can I find a copy of my 1099-SA /5498-SA Form?
- Go to https://www.highmarkbcbs.com/home/ and log in to your HSA account.
- Click on Claims and Spending
- Click the blue Access Spending Account button
- Find the My Accounts box and click the green balance icon to access the Account Summary
- From the My Account Summary select the link Tax Forms.

- Download and save the previous years forms to your computer. Your tax statements are archived so you may go back and review and/or print if needed.
