If a crisis has occurred that has reached a level 3 crisis status within your field team, depending on the severity and urgency, please either call the emergency number or email the Crisis Management Team (cmt@reliant.org) to report the incident. Please include the people involved and a description of what has happened.

Please follow the appropriate Contingency Plan submitted to Reliant for the type of crisis being reported. Reliant will evaluate the situation and communicate next steps.
