The AETNA enrollment form leaves little space for typing in so when we programmed it for Docusign unfortunately many of the headings got cut-off.

 Here Since some of the headings of the Aetna form are blocked from view with the Docusign boxes, below is an example of a filled-out form with instructions for filling in the boxes. . 

Please manually type in information. Pre-filling or auto-filling may cause the form to fill wrong boxes with the same information. Example: pre-filling "United States" in the field for country, may cause "United States" to fill other fields. If this happens, delete the Note: if you pre-fill a field like country with "United States" Docusign will do some weird stuff and fill that in other boxes with that same info, if this happens you may just  have to delete those auto-filled fields and type in the correct info(issue we're working on)information.
