• Go to Google and log in under your Reliant email address & password. Login to a different Google account by clicking the circle icon in the upper right corner. 
• Once logged in, click this link: PMR Template. Upon clicking the link, you will be prompted to make your own copy. Click the blue “Make a copy” button.
• Rename the form (top of webpage). Replace the words “Copy of Sample” with your missionary’s name.
• Rename Question 1. Remove the word "sample" and add your missionary's name.  
• The last question at the bottom is the confirmation page. Edit to include your name and contact info. Your missionary will see this page every time they submit a PMR form. So, feel free to add an encouraging word, a reminder or. additional info.
