The Highmark website will have the Acclaris 1099-SA and 5498-SA form available for download - for the period covering January 1, 2018-December 31, 2018.

• Go to https://www.highmarkbcbs.com/home/ and log in to your HSA account. 
• Click on  "Claims and Spending"
• on Spending. Select the link "Get details, cash statements, tax forms & deposit slips". Access Prior Plan Year"
  • If the link doesn't work make sure you allow pop-ups on this site from your browser.

• Click on Hover over "I Want To..." and select "Statements and Tax Forms"

• Click the link to the tax forms, download and save to your computer.  
  The Acclaris combined 1099-SA & 5498-SA form from 2018 will be available by January 31, 2019.
