Health Choice (2000 or 5000)
With this PPO plan, co-pays for preferred and non-preferred brand-name medications, which had remained stable since 2004 with GuideStone, increased January 1, 2016. Generic drug co-pays did not increase. Specialty prescriptions were reclassified under a new tiered structure, which includes generic, preferred and nonpreferred, with different co-pays at each tier. Participants may contact Express Scripts to inquire about how their prescriptions will be classified.

Cost Savings With 90 day refills - Home Delivery Service and WalgreensThrough the Home Delivery Service, you can purchase up to a 90-day supply of most prescription medications. In most cases the Home Delivery Service provides significant cost-savings compared to filling a prescription at a local drugstore or pharmacy. There may be limitations on some prescriptions, such as controlled medications, subject to state and federal dispensing limitations. Express Scripts now also offers some maintenance medications to be refilled with a 90 day supply at participating Walgreens locations. How to Access Your AccountYour Rx Grp and BIN number are now listed on your medical ID card, and the Express Scripts customer service number is on the back of your card.
