Dependents of Reliant employees covered on Reliant's Domestic medical insurance plan (through Guidestone) are eligible for continuance coverage on Reliant's medical plans under the following circumstances:

  • If the Reliant employee lost benefits eligibility due to a reduction in hours or exited employment with Reliant, their dependents are eligible to be enrolled in continuance coverage.
  • If the Reliant employee becomes eligible for Medicare covearge (due to turning age 65 or  another qualifying reason), their dependents are eligible to be enrolled in continuance coverage. 
  • In the event of the death of a Reliant employee, the decreased employee's  dependents would be eligible to be enrolled in continuance coverage.
