Due to misinformation about the $250 co-pay for ER visits for those enrolled in the HS1500 and HS3000 plans during Open Enrollment for 2019 coverage, Guidestone is extending a limited opportunity for domestic employees that are enrolled in Reliant's medical insurance to switch from a HS1500 or HS3000 plan to the HS5000 plan. This window of opportunity to change must be communicated to Reliant by January 20th in order for the change to be effective for 2019 coverage.

This opportunity is due to the fact we miscommunication that there was no co-pay for ER visits on the HS1500 and HS3000 plans. We were recently informed there is a $250 co-pay for ER visits. This $250 co-pay (incurred for every ER visit throughout the year) does not count towards an employee's deductible or out-of-pocket maximum.

If you selected the HS15000 or HS3000 plan and this ER co-pay fee changes your decision on your plan, you may opt to change your plan to the HS5000 plan. Please email benefits@reliant.org by January 20th, 2019 indicating your desire to change your benefit plan.
