**This is the exact content on our previous Travel Notification page prior to the changes due to the pandemic. We can bring this information back when we change our policy/standards.

For the health and safety of Reliant employees and those they may come in contact with throughout the course of ministry, a Travel Notification Form is to be completed by all Reliant employees traveling away from their country of residence to an international location for their ministry work. Reliant needs to be able to contact you in case of an emergency in your area.

Some examples of these types of travel could include:

  • Short-term projects
  • Visiting another country
  • Conferences or Retreats
  • Furlough (this form should be filled out in addition to a Furlough form)
  • Visa Trips

Please complete the appropriate notification form and follow all steps within the form closely. These travel forms include a request for travel approval from your local supervisor (if applicable) and your Reliant Program Lead, emergency contact information for your destination(s), and an attestation that you agree to comply with health advisories related to COVID-19 in your destination(s) and for your return
