Log into https://toolbox.reliant.org/employment/benefits using your Reliant email and password used to log into other Reliant sites. You can view your current medical coverage in the Employment Section and then Benefits. You can also check your most recent pay stub to determine what coverage and plan you have. Under "Other Information," it should say "Employer Pd (HS2000/HS3000/HS5000)." That will tell you what plan option you have. As for what plan level you have, you can take the amount under "This Check" and compare it to the graph below. |

Employee Only Plan - means only you, the employee, are enrolled in coverage.
Employee + Spouse Plan - means you and your spouse are enrolled in coverage.
Employee + Child(ren) Plan - means you and at least one child are enrolled in coverage. You can have more than one child enrolled in this plan, but not your spouse.
Family Plan - means you, your spouse, and at least one child are enrolled in
coveredcoverage. You can have more than one child enrolled in this plan without the premium increasing

Reliant's job is to make sure you are covered by insurance throughout your employment. We want to make sure we can offer affordable health plans that may not be easily attained elsewhere. You can change your coverage during Open Enrollment effective January 1st of the following year, you will not be able to make changes to your coverage plans without something called a "Qualifying Event." These events allow you to enroll in new coverage, waive your previous coverage, or add/drop dependents to your plan.
If you experience any of these qualifying events, then that triggers a Special Enrollment period in which you can make changes to coverage. During this special enrollment period, you only have 30 days to add or drop dependents.
* The only qualifying event that allows you to change plans (such as going from HS5000 to HS2000) is birth or adoption.

For insurance, a dependent is anyone covered on your plan that's not you. You are considered the "primary insured," and your spouse and child(ren) are considered dependents. Your dependents cannot be covered unless you, the employee, are enrolled as the primary insured. |

Employer vs. Employee Contributions
- Employer (ER) contributions are funds from a non-MTD Reliant account deposited into your HSA account each month. The amount deposited is determined by your plan level (Employee Only, Employee + Spouse/Child(ren), or Family).
- Employee (EE) contributions are optional additional pre-tax deductions from your paycheck deposited into your HSA account. You can select the amount you want to contribute in the 2024 Cafeteria Form.
Both kinds of contributions count toward your Annual HSA Contribution Limits set by the IRS.
