To convert your traditional (pre-tax) 401(K403(b)(9) contributions to Roth (this includes Employer Match, Rollovers, and Elective Deferrals) use this form.

The amount that is converted to Roth will be taxable to the missionary in the form of a 1099 that the employee will receive from Principal Financial Group. This 1099 must be included with the missionary's tax filing for the appropriate year.

To be eligible for a Roth Transfer the Reliant employee must have been an active participant in the 401(k) Plan for 5 years prior to requesting the Roth Transfer.
