How do I access the 1095-B Form
UHA will no longer automatically mail the annual Form 1095-B to employees. One of the reasons for this is because the health coverage information on Form 1095-B is not required for employees to file their calendar year 2019 federal tax return (and future years' returns). However, it is available upon request by calling UHA Customer Services at 808.532.4000, extension 297 or 1.800.458.4600, extension 297 from the neighbor islands. UHA will gladly send the employee a copy by mail or secure email within 30 days of receiving a request.
