• Click on Spending. Select the link "Get details, cash statements, tax forms & deposit slips" (if . If the link doesn't work make sure you allow pop-ups on this site from your browser).

• Click on "Statements and Tax Forms"
  
• Click on "2018 Tax Forms"
  
• Click the link to the tax forms, download and save to your computer.  
• The Acclaris combined 1099-SA & 5498-SA form from 2018 will be available by January 31, 2019.
