Due to misinformation about the $250 co-pay for ER visits for those enrolled in the HS1500 and HS3000 plans during Open Enrollment for 2019 coverage, Guidestone is extending a limited opportunity for domestic employees that are enrolled in Reliant's medical insurance to switch from a HS1500 or HS3000 plan to the HS5000 plan. If you desire to change your planeplan, that must be communicated to Reliant by January 20th in order for the change to be effective for 2019 coverage.
