Employees who are covered by the Reliant group health

plan

plans may be eligible

under certain conditions

for continuation of certain health insurance benefits following loss of benefits eligibility to due a reduction of hours or termination of Reliant employment.

Continuation of coverage generally allows certain qualified beneficiaries to continue temporary access to affordable health insurance in situations where they would otherwise lose their employer-provided coverage.

Continuation coverage only applies to

the

Domestic group insurance plans covered through Reliant

and does not apply to health plans maintained by churches and conventions or associations of churches. For more information regarding continuation of coverage, please contact Employment Services.

(International medical insurance provided by Reliant does not include the ability to offer continuance coverage).

 

Medical Continuance Coverage

Reliant employees covered on Reliant's Domestic medical insurance plan (through Guidestone) are eligible for continuance coverage on Reliant's medical plans under the following circumstance:

  • The employee lost benefits eligibility due to a reduction in hours 
  • The employee exited employment with Reliant.

Dependents of Reliant employees covered on Reliant's Domestic medical insurance plan (through Guidestone) are eligible for continuance coverage on Reliant's medical plans under the following circumstances:

  • If the Reliant employee lost benefits eligibility due to a reduction in hours or exited employment with Reliant, their dependents are eligible to be enrolled in continuance coverage.
  • If the Reliant employee becomes eligible for Medicare covearge (due to turning age 65 or  another qualifying reason), their dependents are eligible to be enrolled in continuance coverage. 
  • In the event of the death of a Reliant employee, the decreased employee's  dependents would be eligible to be enrolled in continuance coverage.
