Before you go to your doctor, download the mail order fax form (This form can be found on the Express-scripts.com website under the Menu option: Benefits - Forms and Cards - subheading: To request a new prescription from your doctor ). Bring the form to your Doctors office and ask your doctor to fill it out and fax it to the number indicated on the form.
NOTE: Only your doctor may fax the completed form to usExpress-Scripts. Alternatively your doctor can e-Prescribe if applicable.
