See below for the steps to take to save and receive a cash reward:
- Get a prescription for a procedure that is included in this program. Some of those procedures include:
- Radiology Procedures
- MRI's MRIs
- CT Scans
- PET Scans
- Mammograms
- Some Ultrasounds
- Outpatient Diagnostic Procedures
- Endoscopy
- Colonoscopy
- Some Outpatient Surgeries
- Carpal Tunnel Surgery
- Bladder Scope Repair
- Other surgeries
- Some Inpatient Surgeries
- Back Surgery
- Hip Replacement
- Knee Replacement
- Other surgeries
- Radiology Procedures
- Log onto the website or call the Personal Assistant Team for options for the procedure and assistance in scheduling your procedure. If you choose a more cost-effective option, you may be eligible for a cash reward. These rewards will be clearly labeled next to the providers available and their estimated procedure cost.
- Have your scheduled procedure.
- Receive a check for the cash reward 60 days after the procedure, should you be eligible.
