The Internal Revenue Service (IRS) requires Bank of America ( the bank that is used by Reliant's domestic medical insurance provider Highmark BCBS for providing Health Savings Accounts to our medical insurance participants) /financial institution that administers our HSA accounts to issue IRS Form 1099-SA if you took a distribution from your HSA during the previous tax year. These forms are mailed directly by Bank of America the bank/financial institution to our domestic medical insurance participants.

What number should I call if I have questions about my 1099-SA Form?

For 1099-SA covering January 1 - November 13th 2017 statement: Bank of America's HSA help line is 1-866-791-0250. 

For 1099-SA covering November 14 - December 31th 2017 statement: Highmark's helpline 1-866-474-0924 (Acclaris doesn't have a direct customer service phone number to call - you'll need to contact Highmark.  Highmark can help you register to access your HSA account online and then they can access your form to send you or they can assist you in how to access the form online).

Where can I find a duplicate copy of my 1099-SA?

• Go to https://www.highmarkbcbs.com/home/ and log in to your HSA account. 
• Click on Spending. Select the link "Get details, cash statements, tax forms & deposit slips" (if the link doens't work make sure you allow pop-ups on this site from your browser)

• Click on "Statements and Tax Forms"

• Click on "2017 View Prior Year Tax Forms"

• Click the link to the tax forms, download and save to your computer.  
• The Acclaris combined 1099-SA & 5498-SA form is available currently, we are unsure if the Bank of America form will also be available or if you'll need to contact Bank of America using the infomation provided above.
