If you fail to resubmit your waiver eligibility, we will auto-enroll you in HS5000, the lowest-priced premium coverage offered by Reliant. The premiums will be charged to your MTD account each month starting in January. However, if we auto-enroll you, you won’t receive the benefit of being able to utilize the HSA savings account. So please enroll yourself in Open Enrollment if you don’t have verification of other insurance coverage.

  • These policies exist due to reasons that are guided by IRS policies and governmental requirements for employers and employees. There are fines that can may be incurred if you or we Reliant fail to comply. We are Reliant is very interested in protecting you from experiencing the financial hardship of any fines.
