If you would like 're recently married or have added a child to your family, you may want to add a new dependent (baby or spouse) to your insurance plan, please send me the attached enrollment forms in an email. Please be aware that adding a dependent may increase your premiums (e.g., moving from Employee to Employee+Spouse coverage or from Employee+Spouse to Family coverage).
