With this PPO plan, co-pays for preferred and non-preferred brand-name medications, which had remained stable since 2004 with GuideStone, increased January 1, 2016. Generic drug co-pays did not increase. Specialty prescriptions were reclassified under a new tiered structure, which includes generic, preferred and nonpreferred, with different co-pays at each tier. Participants may contact Express Scripts to inquire about how their prescriptions will be classified.
