Group Health Plan

Reliant helps eligible employees insure against the potential catastrophic expense of major medical care as well as coverage for routine medical and dental care by providing a group health plan. Full-time employees or those working 30+ hours per week are eligible and required to participate in the group health plan unless their spouse has a comparable plan through his/her employer and can add the Reliant employee to their plan. Eligible employees are also able to enroll their eligible dependents. Part-time employees who average less than 30 hours per week are not eligible to participate in the group health plan. Coverage begins the first day of the month following the month that eligibility began. Specific information about the available medical plans may be obtained from the Reliant website or Missionary Resources. Questions about specific coverage or conditions may be found in the employee's certificate of group insurance provided by the insurance carrier or by calling the insurance company directly.

Full time employees are eligible for short-term disability benefits through the salary continuation model due to a serious health condition that prevents them from fulfilling their job responsibilities. Salary continuation will be dependent upon available funds in the Reliant account for which the employee is responsible. Part time employees who average less than 30 hours per week are not eligible for this benefit. The employee must use at least one half of their accrued PTO if the leave takes place in the first half of the year (from January 1 through June 30) and must use all remaining PTO if the leave takes place in the second half of the year (from July 1 through December 31) before being granted short-term disability coverage. There is a 10 day waiting period before short-term disability begins.  The maximum length of short-term disability coverage will be 90 days, at which time the employee may qualify for long-term disability coverage. Employees who qualify for the short-term disability coverage are eligible to receive 66% of their regular wages and to have their health insurance benefits continued and paid by Reliant. After 90 days, the employee will no longer be eligible for salary continuation or health insurance benefits, except as provided for in the long-term disability coverage and health care continuation coverage. In the event the employee does not qualify for short-term disability coverage, the employee may still be eligible for up to twelve weeks of unpaid leave in accordance with Reliant's Family and Medical Leave policy. Family and Medical Leave would run concurrent with any approved short-term disability leave.

Employees eligible for the short-term disability benefits must notify Missionary Resources as soon as a serious health condition prevents them from fulfilling their job responsibilities. The employee must also provide a physician's statement to Missionary Resources that verifies the nature of the disability prior to the leave of absence beginning. The physicians statement must include an estimate on the length of the disability, or if a return to work date cannot be established, the date of the next appointment with the physician. Missionary Resources will make the determination as to whether the short-term disability benefits will be granted.

In the unfortunate event that an eligible employee becomes permanently disabled, Reliant provides financial assistance through a long-term disability insurance plan. Full-time employees are eligible for coverage. Part-time employees who average less than 30 hours per week are not eligible for this benefit. Coverage begins the first day of the month following the month that eligibility began. There is a 90 day waiting period for coverage to begin once a disabling incident occurs. An employee must notify Missionary Resources as soon as possible when they sustain an injury that may be disabling. Specific information about this coverage may be found in the employee's certificate of group insurance provided by the insurance carrier or by calling the insurance company directly.

Life Insurance

Reliant provides term life insurance that pays a specific benefit to a beneficiary, designated by the employee, in the event of the eligible employee's death. Full-time employees are eligible for coverage. Part-time employees who average less than 30 hours per week are not eligible for this benefit. Coverage begins the first day of the month following the month that eligibility began. Eligible employees may also purchase additional term life insurance through the voluntary term life program. Employees may change their beneficiary by notifying Missionary Resources and completing the necessary forms. Written verification of death within 90 days of the date of death is normally required by the insurance provider. Missionary Resources must be notified immediately in the event an employee dies. Specific information about this coverage may be found in the employee's certificate of group insurance provided by the insurance carrier or by calling the insurance company directly.

Accidental Death and Dismemberment Insurance

Reliant provides an accidental death and dismemberment insurance policy in the event that an eligible employee loses life or limb resulting from an accident, subject to benefit limitations. Full-time employees are eligible for coverage. Part-time employees who average less than 30 hours per week are not eligible for this benefit. Coverage begins the first day of the month following the month that eligibility began. Continuation of Coverage

Employees who are covered by the Reliant group health plan may be eligible under certain conditions for continuation of certain health insurance benefits following termination of Reliant employment. Continuation of coverage generally allows certain qualified beneficiaries to continue temporary access to affordable health insurance in situations where they would otherwise lose their employer-provided coverage. Continuation coverage does not apply to health plans maintained by churches and conventions or associations of churches. Workers Compensation Insurance

Employees who are injured or become ill as a result of their employment may have some of their medical care costs covered under the workers compensation program. Each state has its own workers compensation program. For more information about possible worker's compensation coverage, contact Missionary Resources. Any employee who is injured while performing ministry responsibilities should seek appropriate medical care and then report their injury to their supervisor and Missionary Resources as soon as possible.

Unemployment Compensation Insurance

Some of the states that Reliant has employees located in may allow religious organizations, like Reliant, to be exempt from paying into unemployment compensation, while other states do not. Therefore, employees in the states where Reliant is exempt may not be eligible to receive compensation after leaving their employment with Reliant.
