PPO Plan's Prescription Benefits:
Beginning in 2016, Prescription plan co-pays for preferred and non-preferred brand-name medications, which have remained stable since 2004 with GuideStone, will increase effective January 1, 2016. Generic drug co-pays will NOT increase. Specialty prescriptions will be reclassified under a new tiered structure, which will include: generic, preferred and nonpreferred, with different co-pays at each tier. Participants may contact Express Scripts to inquire about how their prescriptions will be classified. (HC5000 and HC2000 are the two Traditional PPO Plans offered by Reliant).
