2025 Monthly Premium Rates
Employee Only Plan - means only you, the employee, are enrolled in coverage. Employee + Spouse Plan - means you and your spouse are enrolled in coverage. Employee + Child(ren) Plan - means you and at least one child are enrolled in coverage. You can have more than one child enrolled in this plan, but not your spouse. Family Plan - means you, your spouse, and at least one child are enrolled in covered. You can have more than one child enrolled in this plan without the premium increasing. |

HSA Employer and Employee Contributions
Because our medical plan options are all Health Saver accounts, each account is eligible for a Health Savings Account, also known as an HSA.

Once Open Enrollment ends, you will not be able to make changes to your coverage plans without something called a "Qualifying Event." These events allow you enroll in new coverage, waive your previous coverage, or add/drop dependents to your plan.
If you experience any of these qualifying events, then that triggers a Special Enrollment period in which you can make changes to coverage. During this special enrollment period, you only have 30 days to add or drop dependents. * The only qualifying event that allows you to change plans (such as going from HS5000 to HS2000) is birth or adoption. |
