Please include the following information for your card:

• First and last name
• Ministry location (city, state, or country)
• Ministry name (church or org name)
• FD####
• Your reliant url giving link (reliant.org/first.last) 
• A brief description paragraph (60 words max)

Upload this document along with two images where you see "Attach Files". 

Add to cart! Once you've uploaded your documents, add this to your cart. Proceed to fill out your shipping information.

You will not need to provide any method of payment since Reliant will bill your MTD account for shipping fees, and Reliant covers all printing costs for this item. See the billing FAQ below for more information on billing and payment on Chalkline. 

Create a Chalkline account- you will need to create a Chalkline account using your reliant.org email address. Once you do this, you can checkout.

 It's very important that you select the option that says "Reliant FD#". This ensures that your order goes through our Reliant billing process.

Chalkline will then input all your information and send you a pdf proof! Once you approve, this will be printed and shipped to you. You can check the status of your order anytime by using the order number provided or logging into your account.
