  • The bonus request will now be in your Reliant Google Drive. Go to the upper-left corner of the screen to change the file name to include your name and the month and year for the request. For example, Cohort Coaching Bonus Request Form - Jill Missionary April 2022

  • Once you've filled out the needed information, be sure to share it with the MTD Health Team (Jenni.saniuk@reliant.org) for approval. 
  • Once this document is approved, the MTD Health Team will send it to payroll for processing. If this is done before the 20th 16th of the month, it will appear be paid on the next month's paycheck. If it is submitted after the 20th16th, it will appear be paid in the paycheck 2 months paycheckfrom submission.
