If a crisis has occurred that has reached a level 3 crisis status within your field team, depending on the severity and urgency, please either call the emergency number or email the Crisis Management Team (cmt@reliant.org) to report the incident. Please include the people involved and a description of what has happened.
Reliant crisis management team will reach out to the team member who contacted Reliant to walk through the initial ICMT Crisis Report information. Reliant will then also create a tracking log and share it with you and other parties involved securely through OneDrive so that future updates can be documented and communicated to all parties involved.
