Eligible employees and dependents requesting coverage during the policy year must enroll within 30 days of the date of the qualifying event (birth, marriage, status change, etc.) To ensure timely processing, Reliant requests that the employee email benefits@reliant.org (cc international@reliant.org) the completed enrollment form within 20 days of the qualifying event, to ensure we have sufficient processing time to complete the enrollment. Requests for enrollment that are received by Reliant in time to process within the initial 30 days of eligibility will require a Health Statement for coverage consideration. Coverage is not guaranteed and is subject to underwriting approval in order for the enrollment to be approved. |
