Please fill out the waiver on the second page of the Guidestone enrollment form in your Docusign envelope.

For Employees Currently Enrolled in Guidestone wishing to waive:

  1. Complete the waiver located on the second page of the Guidestone enrollment form: Guidestone  Medical InsuranceMedical Insurance Waiver - Guidestone Enrollment Form.pdf
  2. Email the completed and signed waiver form along with a copy of the certificate of coverage or medical insurance card to benefits@reliant.org.
