This form is used to request a church/project fund be created that can receive donations on behalf of your ministry.

- Please note the status at the top of the form will change as the request goes through the Reliant approval workflow process.
- If you list Affiliate Fundraisers, your program team will send links for the affiliate fundraiser applications directly to those emails.
