The PMR is a tool that will hopefully provide a glimpse into the heart. It gives the resident a chance to share the highs and lows of the week and to express any concerns or questions he/she might have. The PMR is customizable, there is a base template, but questions can be added or removed per the resident’s request. For some, this tool will be the primary way to get a look into the heart; for others, you might get one-word answers. Either way, the PMR is a helpful tool and should provide talking points for your individual coaching calls.  

Here is a step-by-step guide to creating a PMR for each of your residents:

***Be sure to make a copy of the PMR  form for each resident. If not, their responses will compile into one, giant spreadsheet and become hugely confusing. It also means you can't share the response page with the worker. Reviewing these responses after a season of support raising can be extremely insightful for a worker.

To access your field staff's weekly responses, you'll first have to set up the spreadsheet where you want those responses to go.

From the Edit Form screen, click the "View responses" button. This will create a new page. This is where you find the responses your field staff will put into their form.
